What to Know
Daily
Check in: 30 minutes before the dive begins at the Extended Horizons Dive Shop, 94 Kupuohi #A-1, Lahaina.
Dive duration: about 5 hours a day for 2 days.
Please bring: Referral form from previous instructor, logbook. We accept most agency certifications and can issue you a PADI or SDI certification on completion. Wear a swimsuit and sunscreen on both days.
Price includes: a boat dive plus gear (dive computer, fins, wetsuit, weights, BCD, regulator, mask).
Do not fly within 18 hours after Scuba diving.
Please select the first of the 2 days on the calendar. In the comment box let us know the date of the 2nd day of diving.
Restrictions: You must complete a PADI Experience Programs Medical Statement/Questionnaire, which includes a medical history section, to participate in scuba diving. If you are a minor, you must have it signed by a parent or guardian.
Scuba diving is an exciting and demanding activity. When performed correctly, applying correct techniques, it is safe. When established safety procedures are not followed, however, there are dangers. To scuba dive safely, you must not be extremely overweight or out of condition. Diving can be strenuous under certain conditions. Your respiratory and circulatory systems must be in good health. All body air spaces must be normal and healthy. A person with heart trouble, a current cold or congestion, epilepsy, asthma, a severe medical problem, or who is under the influence of alcohol or drugs, should not dive. If taking medication, consult your doctor before scuba diving.
Please review this Medical Questionnaire to see if you are able to dive with us. If you answer YES to any question on the medical portion, you must bring a doctor's note stating that you are fit to dive.
